明新科技大学旅馆事业管理系建教合作合约书
明新科技大学 (以下简称甲方)承(以下简称乙方)之协助，合作培育旅馆人力资本，提供学生校外实习机会训练专业技能，从而使学生成为旅馆服务及管理能力的人才，为明确双方之权利义务关系，双方承诺共同遵守下列各项条款：
一、甲、乙双方组成实习辅导小组，由明新科技大学旅馆事业管理系实习委员会
代表暨乙方代表共同组成，实习辅导小组任务如下：
1.召开协调会，解决学生实习问题或讨论双方合作事宜，以利甲方教学与乙方营运的配合。
2.实习期间甲方分发学生至乙方各单位实习，实习学生除接受甲方实习委员会管理外，亦接受乙方实习单位主管之指挥监督，遵守公司政策及工作规则。
3.实习期间双方派专人负责指导与考核，藉以督促学生校外实习的能力训练。
4.乙方负责监督学生实习训练与生活辅导，如遇情节重大情事，得由双方开会议定提前终止实习，并通知家长协同处理。
5.其它有关建教合作校外实习事项。
二、实习期间：自民国年月日起至年月日止。
三、甲方之职责：
1.协助乙方遴选分发实习学生。
2.协助乙方实习训练内容或课程，访视辅导实习学生，提供学生考核资料。
3.通知实习学生接受乙方半年单位轮调安排。
4.负责约束选派之实习学生，切实遵守乙方所安排实习单位工作及作息规定。
四、乙方之职责：
1.提供甲方实习学生员额与专业训练课程，唯不使学生担任非相关及危险性工作。
2.安排甲方实习学生半年单位或职务轮调，并提前通知学生与甲方实习委员会访视老师，但实习学生于柜台、订房、与餐厅内场三项职务得免除轮调。
3.非约定之学生实习单位异动，须征求本系实习委员会同意。
4.实习期间负责实习学生工作时间内之生活管理与实习成绩之考核。
5.实习期间给予实习学生每名每月新台币元正之实习津贴，并依劳动基准法及其它相关法令为实习学生办理劳保、健保。
6.实习期间应给予实习学生比照一般职员之休假与福利制度。
7.其它有关实习事项，依劳动基准法与乙方人事规定办理。
五、本合约如有未尽事宜或变更事项，由双方协调修订之。
六、本合约自双方签约日起实施。

Memorandum of Agreement of MUST and EWM
This Memorandum of Agreement is made between EWM Consultancy Pte Ltd of 171,
Tras Street, # 09-173 Union Building, Singapore 079025 (hereinafter called Sponsor)
and Minghsin University of Science and Technology (MUST)(hereinafter called
College).Both parties hereby agreed to cooperate in the education and training of
hospitality students believing that this co-operation would benefit all parties.It is mutually agreed as follows:
1.Sponsor will provide the attachment opportunity & training facilities to practical for
the students from the College.List of EWM Industry Training Partners are as
follows:
1)The Oriental Hotel, 2) Royal Plaza on Scotts, 3) Le Meridien Singapore, 4) The
Ritz-Carlton Millenia, Singapore, 5) The Traders Hotel, 6) Plaza Hotel Parkroyal, 7)
Negara on Claymore, 8) Swiotel The Stamford, 9) Raffles The Plaza.
2.The attachment of the students is subject to Sponsor obtaining legal permits for the
students.
3.Period of training attachment is 12 months as agreed between Sponsor & College.
4.College undertakes that every applicant is a bona fide registered student of the
College and shall provide for each student the following documentation for
applying the legal permit:
a) Letter of Information in accordance with Sponsor’s format.
b) Latest Academic Record or Certificate.
c) Photocopy of paport.
d) Original pre-deployment medical fitne certificate.
5.Sponsor will provide the students with the following conditions during attachment:
a) Minimum Monthly Allowance of S$600 per month.
b) Free standard accommodation, duty meals, uniform & uniform laundry.
c) Medical consultation by company’s doctor for work related illne or injuries.
Personal medical is students own cost.
d) Supervision and guidance of each student during the training.
e) On satisfactory completion, iue a testimonial and certificate to the students.
6.The College shall be responsible for the following:
a) Recommending selected students based on performance achievement index
and fluent English.
b) Repatriate any students who commit during the attachment period any
criminal offence, seriousmisconduct or violates the rules & regulations of
Sponsor or Employer or if unfit for training.
c) College undertakes to impose severe punitive discipline on student terminated
and repatriated for College shall provide the Sponsor, without any demand,
written information of any disciplinary action.
d) Recover from student & reimburse Sponsor the $5000 immigration Security
Guarantee, if forfeited by Singapore Government, for any trainee who
committed and convicted of Immigration offence.
e) Inform the students their responsibilities during the internship period and their
own expenses for hospitalization & personal insurance and airfares to/from
Singapore.
7.The College and Sponsor shall not be responsible for any misconduct, accident,
injury or death of anystudent during attachment.
8.The Sponsor may terminate the training and repatriate any student at the student’s
expense for any ofthe following reason:
a) Commit any Work Permit offences or involved in any conduct or activities
incompatible or improper with this Training Program.
b) Not complying with the rules and regulations of Sponsor, employer or the laws
of Singapore.
c) Certified medically unfit by Sponsor’s appointed doctor or is incapacitated by
sickne or diseases.
d) Poor performance or conduct.
9.The duration of this Agreement is valid for a period of one and a half (1.5) years
upon signing.During this period, the College undertakes to send students for
Internship or training attachment in Singapore to the Sponsor only and shall not
send any students directly or indirectly to any other party, employer or
establishment in Singapore without the written agreement of the Sponsor.
10.The employment of the Students and this Agreement shall be governed solely by
the law of Singapore.

立同意书签署人__________________与敝子弟实习生(以下简称实习生)
___________________就读于明新科技大学旅馆事业管理系，同意于96年7月1
日起至97年6月30日止，由学校签约之建教合作实习代表EWM Consultancy Pte
Ltd安排第三学年「校外实习」课程之海外专业实习，为规范EWM Consultancy Pte
Ltd与签署人间之权益，签署人与实习生同意下列条款并遵守履行之。
1.实习单位／部门：EWM Consultancy Pte Ltd安排之新加坡国际级观光饭店
2.月薪：600新加坡币
3.每周工作时数：48小时
4.实习时间：12个月
5.实习绩效保证金：实习生应于抵达新加坡后缴交$500元新加坡币给EWM
Consultancy Pte Ltd 作为实习绩效保证金，实习生完成一年实习后将全额退
回；若未完成一年实习，此实习绩效保证金将作为EWM Consultancy Pte Ltd
名誉受损与实习安排经费之赔偿。
6.实习生合约完成之绩效奖金$100新加坡币／每月，绩效奖金于实习完成后一次
发给；未完成12个月实习合约，无法领取绩效奖金。
7.实习期间提供免费宿舍，每间房间住四位实习生。实习生须负责平均分摊每月
水费、电费、瓦斯费及宿舍受损修护等费用；工作时由实习单位提供免费餐
食。
8.若未获机关补助，则实习生自行负担新加坡来回机票费与每日所需之交通费。
9.任何一方若决定终止实习关系，须于一个月前以书面提出或支付一个月的工作
薪资；在实习工作期间与工作之后若有行为不当、表现不佳、触犯法律、违
反规定或与顾客交往，实习单位有权终止实习合作关系且无须提前告知并遣
送回国，由实习生自行负责回国交通费。
10.上班工作时间完全由实习单位安排，实习生可以在同一个工作单位或不同部
门工作。除了实习单位外，实习生不得在新加坡其它单位工作或打工，亦不
得吸毒、酗酒、从事不法、不道德、不诚实、犯罪或政治活动等违反新加坡
法律之行为。
11.除了EWM Consultancy Pte Ltd安排的宿舍外，实习生不可以在外过夜，也不
允许带任何来宾、朋友、亲戚于宿舍过夜；另外不得于宿舍内吸烟、吸毒、
喝酒；实习生应尊重宿舍室友的个人隐私、健康、环境及舒适，并维护宿舍
内的清洁与卫生。
12.实习生须自行负责个人疾病与看诊费用，因个人健康因素离开工作期间不予
支付实习薪资。实习单位负责实习生因工作而引起之相关医疗看诊与住院费
用，医疗福利须完全依照实习单位之规定办理。
13.实习生于新加坡实习期间，由实习单位支付移民安全保证金＄5000 新加坡币
予新加坡政府，若实习生因任何犯罪而丧失实习机会，须完全赔偿实习单位
或EWM Consultancy Pte Ltd之移民安全保证金＄5000新加坡币。另外，实
习生护照将由实习单位统一保管以免遗失。
14.实习生保证完全遵守新加坡法律以及实习单位与EWM Consultancy Pte Ltd之
相关规定。
15.实习生须承诺遵守EWM Consultancy Pte Ltd的相关规定。EWM Consultancy
Pte Ltd、实习单位或同事等不负责个人的受伤、死亡、所有的花费、遗失、
赔偿与支出。
16.前述15条款依新加坡法律管辖执行之。
除了上开建教合作实习代表约定事项外，实习生于实习期间愿意配合学校督
导，遵守贵校各项实习规章及生活条约，服从贵系实习委员会及实习单位之指导
训练，为规范学校与签署人间之权益，签署人同意下列条款并遵守履行之。
1.实习生于校外实习期间，仍为学校学生，优良或不良表现依「明新科技大学
学生奖惩办法」与「旅馆事业管理系校外实习学生奖惩办法」办理。
2.实习生前往实习机构实习，依实习单位请假规定及劳动法办理，若实习单位
无明确规定则依学校请假规定办理。海外实习生应完成一年海外旅行平安险
(附约疾病医疗住院险)事宜，并缴交保险单据复印件予贵系后方赴海外实
习。
3.实习生于校外实习期间，未经告知无故离职、工作懈怠或连续犯错等重大违
规事件，经贵系实习委员会派员调查，会同实习单位确认，经本会通过移送
本校学务处生活辅导组签请学生奖惩委员会处理，视情节得处以下列处分：
(1) 勒令退学。
(2) 记过处分，经贵系实习委员会同意并由签署人具结后，始得继续实习。
(3) 勒令休学，其已实习时数不予计算。

5.Performance Bond: Intern shall remit or pay on arrival Singapore $500/- to EWM Consultancy Pte Ltd as Performance Bond.This amount is fully refundable to Intern upon full completion of Sponsorship Agreement.In case of non completion of Internship period, this Performance Bond shall be forfeited by EWM for lo of reputation and income.
6.Contract Completion Bonus is SIN$100/- per month or prorate payable on completion of full contract period only.Incomplete contract is not eligible for Completion Bonus.
7.Dormitory accommodation will be provided free-of-charge.Each room is shared by 4 persons.Interns shall be liable equally for the monthly payment of water, electricity, gas, and damages at the accommodation.Duty meal is provided free of charge by attachment employer.
8.Daily transport and 2 ways air ticket to/from Singapore shall be Intern’s own cost.
9.Either Party may terminate this Sponsorship Agreement by giving one (1) month’s notice in writing or payment of 1 month’s allowance in lieu.The employer is entitled to terminate the Sponsorship Agreement without advance notice and repatriate the Intern at Intern’s own cost in case of misconduct, poor performance, breaches laws, regulations or fraternization with guests or company’s clients during or after work.
10.Working schedule is strictly according to employer’s schedules.Intern may be attached to same section or department for the complete 12 months at the Employer’s sole discretion.It is illegal for Intern to work with any other employer whilst in Singapore and illegal to take part in any drugs, alcohol, illegal, immoral, dishonest, criminal or political activities while in Singapore.
11.Intern shall not stay at any accommodation other than the accommodation provided by the employer or EWM and not allowed to bring any visitors, friends or relatives to visit or stay at the accommodation.Smoking, drugs, and alcohol consumption is prohibited in the accommodation.Intern shall respect the privacy, health, environment and comfort of others living in the accommodation.Intern is required to undertake cleaning and maintaining the accommodation in clean & hygienic condition at all times.
12.Intern shall be responsible for own illne and outpatient medical treatment.Training Allowance is not payable during medical leave.The employer is responsible for work related medical treatment and hospitalization only if it is caused directly from the employment with them.Medical benefits shall be solely in accordance with employer’s policy.
13.Intern shall fully reimburse the employer or EWM the Immigration Security Bond of SGD 5000 if it is forfeited due to any offenses committed by the Intern.Paport is kept with Employer to prevent lo.A deposit of$5000/- is required by employer for Trainee to keep it.
14.Intern undertakes to fully observe and comply with the laws of Singapore and the prevailing Rules and Regulations of the Employer and EWM.
15.Intern hereby undertake to keep EWM Consultancy Pte Ltd, it’s Principals, employees, agents and sub contractors indemnified and to hold them harmle against all actions, proceedings, claims, demands or liabilities whatsoever or howsoever arising which may be brought against them or incurred or suffered by them or arising out of or in connection with the performance of the Agreement, against and in respect of all costs, loes, damages and expenses.
16.This Agreement is governed solely and exclusively by the laws of Singapore.
