1）申根签证申请表（英文版，和中英对照版我都发给你，但是填写得填英文版的）
一张用英文填写完整的申请表并由个人签名。 2）一张彩色的照片（不能和上次用一样的！！！重新找一个，或者你自己照了ps一下尺寸去印出来，随你，但是总之不能和上次一样）
护照照片大小（47mm ×36mm）的浅色背景正面近期照（不超过6个月）。 3）护照（或其他有效国际旅行文件）及一份护照身份页的复印件。
